STONY BROOK FINANCIAL ASSISTANCE PROGRAM: Financial Assistance is available to patients who have limited income, have no health insurance or are underinsured. You may apply for a discount regardless of immigration status. The amount you pay depends on your income. Nutrition Programs

Meals on Wheels – There are thirty-one Senior Nutrition Programs throughout Suffolk County. Home delivered means are available to eligible homebound older residents age 60 or older. A client assessment is done to determine eligibility. For further information on the meal program and be provided with your local Meals on Wheels site, call: (631) 853-8227.

Nutrition education and nutrition counseling are available to all who are 60 and over by calling phone # (631) 853-8232.

Respite Services
Respite Care provides caregiver of the elderly with temporary relief. The suggested contribution is based on ability to pay. For additional information call Suffolk County Respite at (631) 321-8229 Ext. 1229

Transportation

SCAT Transport: (Suffolk County Accessible Transportation) – Provides curb to curb services to individuals with disabilities. For applications and information call: (631) 853-8337 (voice) or (631) 853-5658 (TTY) weekdays, 9:00am – 4:30pm

LogistiCare: For individuals that have Medicaid and Managed/Medicaid with transportation benefit, please call 1-844-678-1107 to schedule transportation to your doctor’s appointments.
